Supporting Better to deliver London’s new Urgent Care plan

ReStart are delighted to be Better’s integration partner for the successful delivery of London’s Urgent Care Plan.

Providing digital care services to 11 million people, the revitalised service now allows Londoners to have their support wishes digitally shared with every health and care organisation in the region. The Urgent Care Plan is led by OneLondon whose goal is to join up health and care settings across London.

ReStart provided a central integration engine which facilitated the migration of 52,000 end of life care plans, enabling the sharing of PDF plans across all five of London’s Integrated Care Systems, and also building three query interfaces ensuring urgent care providers can check for the existence of a care plan before critical decisions are made.

The complex migration strategy was quality controlled and audited at every stage through the integration engine by ReStart – every single piece of data in each active care plan was carefully migrated from the incumbent system.

As a result of the integration work by ReStart, The London Care Record now has visibility of all care plans in PDF format to ensure patient wishes are always adhered to. Furthermore, if a patient withdraws their consent to share data in the Urgent Care Plan, the integrations ensure their wishes are updated in real time to all downstream systems.

As we move into the next phase of the project, the ReStart support service kicks in to proactively manage the interoperability flowing through the integration engine. Our bespoke ReStart Alert Monitor (IMX-RAM) tracks system heartbeats, detects queues and raises alerts, helping underpin continuous safe care for London.
